Why do some players have a face gen picture on the field (and if a pitcher, on the scores page) even though I loaded a photo that appears on their card?

Cause you don't have your pic settings correct or not updated to latest patch (see note below).
OOTP defaults everything to facegen..so the first thing you need to do is click the File tab and select settings. In the popup, click the facegen tab and set the last option to search for photo first. Restart ootp for the settings to take effect.
Once restarted, click the Game tab and select Game Settings.
On that page, click the Players & Facegen tab and on that screen on the right hand side, Uncheck load photofit (facegen) files automatically.
This is not going to help your saved game though, so follow these steps as well as the above:
Goto your saved games\news\html\images\person_pictures and profile_pictures folders and delete everything.
Restart ootp and it will auto rebuild the real pics...that should fix your problems..
Note: Best to be using the latest version of ootp..18.2.8 as there was a bug with real pics in previous versions.
