Assuming it was a runner being doubled off third, the wheel play wouldn't be used (that's only with a runner on second or first and second). So this should almost assuredly be 2-5. Probably a typo in the play-by-play and hopefully recorded correctly in defensive stats.

I would say 2-5 or 2-6. If the hitter was bunting, then the 3B is maybe playing in and the SS was covering the bag.

Possible, but not likely. The third baseman typically will play even with the bag and only charges only if the runner on third breaks first. Otherwise, if the third baseman charges, the runner can follow him down the line and have a much better chance to score. The shortstop could circle in behind, but that's a rotational play that's more common in college baseball than in the majors. Because of the high level of play in the majors, teams are usually unwilling to have that many guys out of position on a potential bunt, especially with how little the bunt is used in the current game.
