Great Midwestern League
This will probably involve knowledge people have from OOTP20, but I imagine it will be the same in 21. It is an idea I started playing with in 20, but got distracted by other shiny objects and never went through with it.
My idea is a Major League, without minor leagues, and then 5 or 6 "State Leagues" at AAA, or maybe AA level, independent, but in an association. The top league will be able to buy players from them. My questions are:
Fired up for OOTP21 and the new MLB season, so this idea came to mind as one that I never completed in 20.

1. You should be able to use templates. Go to the Advanced tab in the initial league setup. Setup the settings you want and then save it as a template. Then load the template change league and team names, repeat.
2. You would have to do this manually. 3. The league level doesn't matter as much as the Player Creation Modifiers. You might even want to set theses leagues as independent leagues(if you don't want them referred to as minor leagues) and set the PCMs to a level that works (you'll have to experiment with that, start with the same as AAA maybe which is .850). 4 You can set the fee, but there's no setting for time of year. Don't have any experience with this setting, but I'd assume they could purchase players at any time of year.

Its a reference from a time when the east cost was all that counted and the West was considered to start at the Mississippi.
