I have my fictional league set for 145 games seasons, but teams actually play between 115-120
Yeah, the teams play different numbers of games. I have the current MLB setup (two subleagues, three divisions, five teams each, etc.) I've disabled rainouts, turned off the balanced schedule, enabled interleague, deleted the schedule and generated a fictional one. Nothing works. Each team has this weird randomized stretch of 5-6 straight off days, but even if they didn't they still wouldn't be at 145 games. I'm lost.

I don't think it's a bug. The schedule generator can't handle certain unusual schedule setups and lengths like your 145 games. Go to the schedule subforum and see if there is one for that amount that somemone already created that you can use. You might be out of luck but you could find one that is close enough.

Also, if you have balanced schedule selected, try deselecting it
