3 Batter minimum?
So what exactly is a 3 batter minimum?
a) Pitch to 2 batters then 1 pitch to a 3rd? b) Pitch to 3 batters requiring an official plate appearance out for all 3?

Face (pitch to) three batters. I don’t think you can switch pitchers during an at-bat anyway right?
Edit: if a pitcher finishes an inning, that clears the requirement too, even if they only faced 1

You can change pitchers in the middle of a plate appearance, but it is rare. I have seen it happen when a guy just loses command, and they eventually bring in a strike thrower with a 2-0 count on the batter,
And of course people can get injured. I hear the Astros are giving acting lessons to their staff during the time off. |

Simple fix to that one - if you remove a pitcher from a game due to injury and he cannot face the minimum three batters, then you must immediately place him on the disabled list.
That'll stop shenanigans

It's an interesting question for sure. I think the key word is "faced". Now when most people think of the word faced they probably just think the pitcher maybe only necessarily pitched one pitch to the batter, if that (someone might also interpret it as a pitcher "faced" the batter before they were pulled without having pitched to him). However, at least when looking at the "batters faced" statistic, I think it's clear that MLB considers "faced" to mean you got a PA out of the hitter. So I'm thinking it's the latter.

so, according the official rules of baseball update, the wording indicates that you must complete 3 batters (either they reach or are retired)
