4 on 4,5 on 3
I only see 4 on 4 hockey in OT and never 5 on 3. Also,no matching minor penalties or double minors. In most cases the game plays very well. Are changes going to made to incorporate the suggestions I'm making. One more thing, the computer never replaces the goalie with the extra attacker when down by a goal or 2 in the last 2 minutes of regulation.

Are you setting the tactics to pull your goalie in the last two minutes just before you start the 3rd period? If so you might want to check your lineup between the second and third period. I've found that though I set my lineup prior to each game I play, including the players I want as Extra Attackers, it does not save them when I actually begin to sim a game. I have to go back into the lineup window prior to simulating the game and add the players again to the EA boxes. Hope this helps!

it's also possible to have a 5 on 3 in OT, albeit rare. A team has to take two penalties for that to happen. Instead of 4 on 2, it becomes 5 on 3.
