Duplicate Players in Player Alphabetical List

I don't have duplicate players, but I have duplicate listings that link to separate player reports in the alphabetical list. Any idea why this would happen or how to fix?
It looks like one of the players versions is a free agent, and the other one is assigned to a team.

Oh wait...somehow I do have a bunch of duplicate players. How the heck did that happen.

How often has it happened, and is it still happening?

I'd be curious if anyone has an answer to his question. It happened to me too with Troy Percival in my historical league. I signed him to a FA contract, only to find a duplicate of him still in the FA pool.

I "retired" the duplicate, and haven't seen others yet, but curious if I should keep checking on it.
