Random players added to my team

I do not know if there is a post for this already, but, in OOTP 25, I was going through the season and when I was going to the next day, the game told me that I had a player designated for assignment and he had cleared waivers. I did not trade for this player or sign him. He just randomly appeared. I released him. But that worried me a little.

Next, I had a player on IL but when I checked the box score for a game I just simulated, he showed that same player, who is on my IL, in the starting lineup. Now he did not have an at bat and had zeros a crossed the board, but that also worried me a little bit.

Has anyone else noticed these errors?

I think I know where those players are coming from. Could they be players taken from my team during the Rule 5 draft and they are being returned to me?

I released the player.

Basically, it was a player just randomly showing up in my DFA box

The boxscore was just a glitch but it did not affect anything. The player on the IL had zero at bats, hits, strikeouts, etc. And had a player substitute for him below the IL player. I am not too worried about that but the random player in my DFA box worried me.

What did the team transaction log say or his history page say about it?

I researched it and found out that it was a player given back to me that another had taken from me during the Rule 5 draft
