Where do I get one of these? Guys like erickjaffe and many others are wearing them as avatars.

It started as an Off Topic Baseball League thing but being the trendsetters that we are, it caught on fast. First stolen by the copycat OTBA (of course) and now spreading like wildfire across the entire OOTPD!

.....but where were the fonts found? Those numbers/letters seem to be perfectly matched to each team.

Eriq provides the fonts. You can find them on his website. I don't know what it is off the top of my head, but I will look it up for you later.
