Questions regarding changing country and city data after starting league

So I'll try to explain what I'm asking, and see if this is possible without making a mess of things.

I'm planning out a league that will span over a century. During this time in real life, countries change names, cease to exist, and are created. Same with cities. Does the game load the files that contain this data and never reload it again, or is there a way to have the data be editable without affecting data that's already in the game (aka if I change Country A to be Country B, it won't change all entries of Country A to Country B)

Bouncing a very old thread, but wondered if anyone has ever ran into this, or has an answer?
