Double Steal Success Rate
Very small thing, but has anyone else noticed when you play in one pitch mode, the double-steal success rate has to be nearly at 100%? I try not to exploit it, but when I have my 1-2 hitters on first and second with pretty good speed, I'll double steal and I feel like it works about 95% of the time. Anyone else? Not a huge problem at all, just something I noticed.

I've noticed this as well. I try not to exploit it, but sometimes a double steal is the best move at the time.

I've also noticed when trying to steal second with less than 2 outs and a full count, it seems like my batter will always strike out looking.

I don't think I've ever seen my batter swing at a pitch when I called for a stolen base. I've been playing the game since 2001. I literally don't think I've ever seen a batter swing at a pitch when I called for a steal. I stopped calling for steals with 2 strikes years ago.

This looks mostly correct to me, but I seem to remember the batter swinging sometimes on a "send runner". If that is the case, I don't know the difference between "send runner" and "run and hit".

I guess the only distinction between "steal" and "Run & Hit" would be whether or not the batter has the option to swing. So the existence of both suggests "steal" means "don't swing." Assuming that's true, the circumstances in which you would select "steal" are very limited. Like, you have an awful hitter who hits tons of ground balls and Vince Coleman is on first base representing the winning run with no outs.
[Edit] Actually no. As I read the "run and hit" explanation, the runner goes no matter what. That's not what I want to do either. I want to give the runner the green light to go if he thinks he got a good jump, and I want my hitter to be free to swing. If "steal" means the batter never swings, I don't see that I have that option. And I absolutely should have that option. In real MLB, most of the time when a runner has the green light to run or not, the hitter is free to swing if he likes the pitch.
