What are the problems with double-headers?
Do double-headers cause problems with the game? Are there any quirks I should be aware of?

What happens if the first game goes into extra-innings? Does OOTP play 2 games simultaneously or does it know to wait to start game 2 after game 1 has ended?

Did a test:
Got a great test result too. Extra innings and a rain delay: And an injury: To my pleasure game 2 started after game 1 ended. The injured player was pending diagnosis for game 2 and the pitchers used were tired. Very happy with this result. And as an added bonus the weather made sense for game 2 (hopefully this was not a coincidence) |

The answer to your question is no, there isn't really anything inherently wrong with double headers in OOTP. In historical play, I keep them in all the time.
The only kinda problem is that the AI doesn't really plan for them very well (i.e. like call up an extra pitcher). But I don't think that is a real surprise since the AI is pretty reactive rather than proactive all the time. |

Good to know. I can live with bad AI planning because I already live with bad AI planning.

Doubleheaders work, but they could be handled better. OOTP's interface doesn't identify doubleheader in the schedule either before or after they are played; if using the RTS feature you won't get the start time of the second game being thirty minutes after whenever the first game finishes; and OOTP doesn't distinguish between regular doubleheaders and split, day-night doubleheaders.
