I'm not sure if this has been mentioned or not, but I'll give it a go:

Each different League within the universe should have a different rating based on caliber of player. For Instance, the Japanese League generally has players equivalent to AAA players, or fringe Major League players. It would be unrealistic if Japanese teams were just as good as MLB teams. Also, you could create a completely fictional league with players equivalent to rooke league players, single-A players, etc.

Also, the goal of the manager could be to move up. Say you start out in Rookie ball, but after one season an Independent team offers you more money. You could leave, but then your chances of moving up into AA, AAA and so on aren't as good. The goal is to get into the majors.

One more thing. There should be different goals depending on where you are managing. Such as: Developing players, income, winning, team happiness, owner happiness. If you are a double-A manager and there is a 19 year old phenom SS who is struggling, your main goal may be to develop players, in which case you keep him in the lineup.

Or say it's June 21st, and yo uare the manager of a big league team. Your owner may request you start dumping salary, but you still think you can make a push into the playoffs. If you don't do what he requests, and don't make the playoffs, you'd be more apt to be fired.

Just some thoughts.

I hope this is taken into consideration as well. Obviously there has to be the occasional Ichiro, Matsui (even the busts like Kaz) and Iguchi.

obviously there would be players who such as Ichiro or Hideki Matsui. My biggest concern with the fact that there are different league within the universe is all leagues aren't created equal. I'd hate it if the MLB (or whatever league a person chose to use) got watered down because players started joining other leagues.
I'd hope you could structure salaries different in each league. For instance, you could create a league where all salaries were at a standard $50,000, or create minimum, maximum, and average salaries.

I'm 90% sure this will all happen. In SI's other games, at least, the relative level of competition from one league to another is modeled very well.
