Odd Pinch Hitter Bug

Something odd just happened.

Billy Wagner has pitched 4 Innings in releif (they used all of their MR's), and his spot was due up to hit. They PH David Bell for him. Then the next inning, Wagner is still in the game, but the lineup lists David Bell in his spot with no position. Well, entering his 6th inning of work, and going over 100 pitches, Wagner, magically still in the game, gives up 5 runs. Bottom of the 14th, that spot comes up once again, and David Bell is still there, and grounds out.

Can someone explain .... ?

I've noticed the above problem as well. I believe it happens when the computer team has run out of position players. When that happens, the pitcher stays in to pitch, and the PH stays in to bat.
