If you have a second nationality in the player profile, does the FACEGEN create a different picture other than the one without?

I don't understand your question. If you have facegen on, the player will only have the one facegen, which will update when he switches teams (or moods if you have that option)

The way I took the question, will the Facegen for "Bob Smith" USA be the same Facegen for "Bob Smith" USA and ITA. Put differently, does having a second nationality alter what the Facegen looks like.

If that’s the question, then probably not. Facegen is set by the ethnicity/facial type you can see in the editor (Caucasian, Hispanic, etc.). Adding a second nationality wouldn’t change the facegen. Having a second nationality might influence which facegen shows up, but would still fall in one of the ethnicity categories.
