So, Minor League Free Agents Can Just ... Bolt??

I'm in an online league on OOTP 23. I have had this one guy for seven seasons. He's a slow developer so I needed him to cook for a couple more seasons, but then he should be really good.

I offered him an extension for next season as a minor league free agent and figured it was pro forma. But next thing I know, I update the game, and I've been informed this minor leaguer has signed up with another franchise, and interest in my team among my fans has crashed!

My question, is, how can I offer him an extension, be told for several updates that it's pending, and all of a sudden I learn he's just gone? Shouldn't I be getting a message from him saying, extension offer is insufficient, you need to make a real offer to keep me? Or is that not how it works in OOTP, or at least version 23?
