Pick off in box score
Although MLB box scores list base runners who are picked off when on base, the OOTP box score does not do this. Might this be a good thing to add to the standard box score?

Yes sometimes but not every pickoff is an attempted steal so another notation should be used.

Yes, a pick off COULD be listed in the caught stealing category, but it is not. In the game I viewed, each team had one runner caught stealing, and those were listed as such. But the pick off was not listed or noted anywhere, except in the game log.
