Quite honestly, this doesn't even seem all that far-fetched to me considering the way that managers handle pitchers these days. Someday soon the almighty pitch count will eventually come before all else, including no-hitters and perfect games. Maybe OOTP is simply clairvoyant in this regard.

I think I'd rather throw it in underhanded instead of using my other arm for fear of looking like I'm throwing like a girl, which is what most of us would look like if we threw with our off-hands.

Surely in the coarse of the next 161 games we could have recovered from an opening day loss. The only argument I can make for pulling him is that he is by far the best pitcher in the league, and its probably not a good idea to play around with injuries. Still, if I had been given the choice I would have left him in. This brings up a good point. I am sure as it currently works, the injuries aren't determined until after the game, but if a player get a very minor day to day injury during a game, it would be nice to be given the option of keeping him on the field. Yeah, no way you pull the guy with that kind of lead, IMO.
If a player gets any kind of injury, it'd be a great addition to the game if there were a pause and information disclosure with details. If it were a minor injury, a DTD that allowed for keeping him on the field, then having that option would only enhance the gaming experience. A version of visit the mound, but a Trainer to the Field.
