Another Pitcher AI Mismanagement

It was in the bottom of the 6th in a close 2-2 game. Castrovalva had 2 outs but they had the bases loaded with a tired pitcher coming to bat. At that point in the game he had thrown 107 pitches and I had the "Hook Starting Pitcher" set to maximum (but at average for the 6th inning).

Not only did the AI not put in a pinch hitter, it left the pitcher in the game until the middle of the 8th and let him load the bases with no outs - and by that time he had thrown 134 pitcher.

It seems like you can set the team strategy settings, but the AI pays absolutely no attention to it. When will the AI learn how to set lineups & pull pitchers?

ATTACHMENTS:
1, Pitcher (Dipre) at bat (having thrown 107 pitches) with the bases loaded
2. Status (at the time of the AB
3. Strategy Settings & game results (134 pitches)

I will admit that the Strategy Settings (for Hook Starting Pitchers) for later in the game were at the middle (default) range, but even then - 134 pitches?

What's the strategy for Innings 4-6 in a tied game?
