They're two different things. On the pitcher side it's likely a slow delivery but may include a poor pickoff throw. As an in-game option it may affect pitcher command/control. If your pitcher has poor control avoid hold runner for sure IMO. Throw over while understanding the risk. If you have to; pitch out. Not my cup of tea either.
