This is what I always do.

1. Add player photos to: Documents\OOTP Developments\OOTP Baseball 20\photos.
2. Download and install the Historical Facegens pack.
3. Create new league.

Depending on your 3D & Facegen settings, the game will look for a photo first or a FG image first. You should not get a mix of real photos and FG images, unless for some reason the player does not have a real photo or FG image (one or the other). But the comprehensive photo pack and historical facegens pack make that unlikely.
