Afaik, only MLB has an actual monopoly exception, based on a ruling from the 1st half of the 20th century. Even that has been worn down by subsequent rulings through the years.
The key factor allowing them the ability to set rules on player movement, salary limits, etc. is that they have a CBA in place with the players. That is why one of the steps the players can take during a work stoppage is to de-certify the union, because with out a union representing the workers, full ant--trust rules come into play.
