The transactions file never used to include injuries. Now it does. If your issue has nothing to do with confusing the game engine by enabling injuries while using historical transactions, then you definitely have a problem.
Personally, I haven’t seen this kind of problem while testing the transactions file in OOTP20 with injuries disabled.
