Do the Custom League(s) still mimic league size as the NHL or can you start off the bat with larger leagues?

Do the minors expand along with it's major league or can it have a separate expansion?

Any chance we will see custom leagues allowed in the real world(NHL/AHL /KHL) anytime soon?

My memory isn't that great so hopefully I didn't ask these questions earlier.
