There's a bug that prevents names from duplicating onto multiple namesets which has existed since OOTP18. I mainly notice this on templates, but if it's showing up away from them, that's important news to post in the bugs forum. That said, and I don't mean to throw the devs under the bus for saying this, I've tried twice to bring it up in that forum (this year and last year), and have heard nothing from them.
I still hope it can get fixed, but I don't feel comfortable with posting that bug report there anymore. I'm just relieved someone else has noticed this too.
