I've seen 16 ERs in 4 inning once and 12 at least 6 times. This is an obvious bug on using multiple relievers before the 6th inning.
This league has had no other bug probs and is running on a 3.2 GHz P4 w/2 gigs, etc ... I have everything on: all logos, historical stadiums, all player photos, sim minors, game logs, keep all box scores. Well, ok, no sound, as usual though I wish I could without risking crashing my league...

As a workaround, check in Team Strategies and set the "pull starters" all the way to the quickest setting (all the way to the left on the slider) and set "pull relievers" to the less or slowest setting (all the way to the right).
In addition, go to League Setup and find your Era settings...there are settings there where you set the use of starters and relievers...sorry, I can't get into the game setup and give you more specifics...my game is down and I have spent about 18 hours trying to get it relicensed. Nothing with computers is simple for computer-simpletons like me.
You can also set pinchhitters settings in Era settings and in Team Strategies. This will cut down on pinchhitting and pitching changes, too...or in your case, increase it so a pitcher is removed quicker.
Hope this help some. It helped me stop starters from giving up 10-12 runs before the AI would pull them...but it still pulls relievers too much and too soon.
