The first one you listed, about the missing team name in the news entry, is a well-known and quite annoying bug that has been around since OOTP5 or earlier. Here's
a thread I started on it over three months ago.
The second post, about one team being one-third of an inning short, is harder to figure... I see that the same team's LF has an error, and there are a number of problems that still come up when there's a sacrifice fly & wild throw to the plate, so if you have it, check the game log and see what happened on the LF's error.
