Check all the lineups/depth charts/pitching rotations looking for stuff that's invalid. (I.E. players in the lineup but missing a position) Check the minors as well... some have found that invalid minor league lineups may be a cause for this. (You can check this really quick by turning off minor league sims... if this fixes the problem than that was where the issue lies...) Make sure the minor leaguers in the lineups have positions...
