I had this happen when I created a second "user account" on my PC. The "second" user had nothing to do with OOTP. After a period of inactivity, Windows would detect this and would return to the Windows "user logon" screen. Once that occured, I could never get OOTP going again -- same as you described.
I deleted the second user and have had no problems since then.
Strange, eh?
