Might it be your schedule isn't providing enough off days for the pitchers to rest in between starts? I would think that without a sufficient number of days off to rest, it's not going to matter what size rotation you select, as the teams are going to have to throw in other pitchers to fill in when the 3 main starters are tired.

For a "perfect" 3-man rotation, that is, each of the three starters gets 3 or 4 days off between starts EVERY time so that a spot starter need never be used due to fatigue, there can be only 5 games per week. Having games only on, say, Tuesday-Wednesday-Thursday and Saturday-Sunday would work. Using this pattern of games every week means the three starters get 3 or 4 days off between every one of their starts.

On the other hand, it may be a bug, I can't say. I just thought I'd mention another possibility to consider.
