So again, I believe the game generates a number 0-255 then scans through the injury.txt file looking for injuries that have that number. If that's true, that means basically any time an injury occurs, there's a 1 in 256 chance of it being career-ending.

That would be unfortunate. It would be nice to adjust those frequencies even for more serious leagues to simulate advancements in medicine and conditioning or to simulate more primitive medicine.
