The trouble is, when you're in the 9th, you're not expecting the game to go to 25 innings. The manager is thinking someone will score a run next inning maybe, so he uses the bullpen because it might only go for a couple more innings. It's a tricky decision to say "I'll bring a starter in now" when you have no idea how long the game will go.
