See the longest game post.. but to paraphrase:
Pitcher hitting or running:
Click on Pitcher
Turn on Editor
Change Position to anything other than pitcher
Insert into Lineup as hitter or runner
Return to player editor and make player back to being a pitcher
Position player pitcher:
Click on Player
Turn on Editor
Make him a Pitcher
Insert into game
Return to Player Editor and change position back to normal.
