It's good to troubleshoot the obvious things first:
1. Are you using the correct OOTP History version for the correct OOTP game?
2. Did you use the commishJoe utility from Jeff cato's website? The most recent one for the BOSI pages won't work with the last version of OOTP History.
3. Did you output the required OOTP html pages?
4. Did you input the correct path within OOTP History to the CSV files and html files you generated above?
5. Team Names cannot end in a period. For example, ‘St. Louis’ is okay, but ‘N.Y.C.’ will crash the program.
6. Divisions must have names, even if there is only one division in each league.
7. Are there two teams with the same name or abbreviation? Example: 'NY' and 'NY'.
Sorry if the above seem too obvious, but most often the OOTP History errors are things like that, check through them anyway and maybe one of the items applies to your error.

You can use it at the beginning of any year but if you do not do a CSV export (commishJoe's util) and an OOTP HTML export
each year then you will only be getting the OOTP History pages for the year or years where you did export the CSV and html files. For example, in your case you would be using the 2014 CSV pages generated by CommishJoe's utility plus the 2014 OOTP html pages generated by OOTP and OOTP History would in turn just output history stats for 2014. It would be like the league began in 2014. Note that lot's of online leagues that have not kept their previous year end league file for each previous season have still used OOTP History. It just means you miss out on the leagues earlier years.

This will not matter if you are happy beginning the OOTP History pages in 2014 like it sounds like you are trying to do.
