To create such a storyline, you could edit the html that has the storylines and edit the file to make such a storyline on your own. Or edit the injury file (IIRC it is a txt file), and change some injury to death by whatever.

There will never be a storyline that results in a player dying. Years ago, Markus mentioned that OOTP would include player deaths (so that, for instance, you wouldn't have 150-year-old retired players still hanging around in your database), but that never happened and I think that's now completely off the table since OOTP entered into a licensing agreements with MLB and the MLBPA. So yes, if users want storylines that kill players, the only way to do that is to edit the HTML file themselves. Even then, though, players can't die, they just retire. The only people who die in OOTP are owners, which I take as a subversive commentary on late-stage capitalism.
