This was a major issue patched into the game at the end of 25's life cycle. Hopefully, we can finally fix it on 26.

Here is the order of events:

I offer a player 6.9 million
We sim a week
Another team counters with 11 million
We sim a week
I counter with 12.4 million
We sim a week (currently here)
Player says I offered 6.9 million and I need to offer 12.4 million to take the lead

The player is now saying that my last offer of 6.9 million is not enough and he wants the exact amount of money I offered him the previous sim. If I click on any of the + or - buttons, he now says that my team (Houston) is leading the bidding and that I can increase the offer if I want to. But if I back out and go to pending offers, it still says he wants more. And if I open up the contract, he is then back to saying I offered 6.9 million and he wants 12.4.

Of the 50-ish offers I have out there three weeks into the offseason, six of them already have this bug. When this happened on 25, most of these players would not sign any of these bugged contracts and would become increasingly frustrated, cutting off negotiations and insisting we were offering them outdated offers and lowballing them.

Really hope this can be fixed soon, as we hoped when we converted the league from 25 that this game breaking bug would be solved.
