At the Start screen, go to "Settings" and go to the Troubleshooting tab and turn on Trace Mode (Write OOTP debug into file) and when the crash happens, go to the adjoining button and open the txt file. Save it and post it so they can see where the game is crashing. Whatever it is maybe forcing off the facegen and other items.
Matt or maybe Will can look at it. It's a small team of developers and they themselves are best equipped to see the issue. There have been odd 3D crashes in OOTP26 that have been under investigation. Nothing precisely like this though based on what you wrote.
