Question I have is how this would affect historical players on their nationality when they get imported into the game. Would their nationality change depending on which side of the country they were born or have any nationality at all if the old USA nation id is no longer there.

It would change their nationality immediately to the new country when you reimport the world and names file, but you could get around this by making your new nation have 100% dual citizenship with the old nation, it won't change the existing guys but it'll make sure anyone new coming in does akin to Puerto Rico/USA.
It's better to do this in a new league than in an existing one for that reason, though lots of ways it can happen I guess akin to a new nation being born of an old one (South Sudan from Sudan)
