Okay, so here's the thing with Designating a player for Assignment.
This is exactly what I was thinking when you showed this the first time.

One reason a player is DFA, is because if they have a major league contract, and you have no room on your active ML roster, then they go into the "DFA" category as part of the roster update process. You must then manually assign the player to a team within your organization. It has 10 days to move a DFA. It's actually impossibly easy to avoid anything like this Waiver problem to happen.

The moment your trade is completed, or your player signs a contract from free agency, immediately add them to your Major League roster. This removes them from DFA status, within their 10 days, and your problem is solved.

As for Waivers, that's different. There should not be a reason a player should be waived the moment they are signed. That has to be either in your league settings (which I can't imagine a reason why it ever would be) or that itself is the problem here. Waivers are 3 days generally, so this would be assumed that they're instantly waived upon signing. That can't be right. But it would again be avoided if added to the roster the moment they sign their contract.

As suggested by the dev, as well, how much active roster control do you have over this team? If you have full player control this would not happen as well, but if the AI has control over your roster changes, then that would play a MAJOR impact on the way your day-to-day team operations work. This would cause you to have to severely increase your oversight over your franchise's operations, to make sure the AI doesn't do anything incredibly ******ed like this. That would absolutely be the first place I would say to start. Remove the AI from your franchise's roster control decisions. Then you'll have better control over your roster, and a TON less headache like you're having, and have a hundred different things function more the way you want them to.

But start with your DFA problem, get them on your roster, and don't advance a week with them on the DFA (like is in your image, where it says "5 days left"), and things like that. You definitely don't want to simulate further after making a trade or a FA signing without making sure they are where they should be on your roster.

I hope this helps clarify a few things, and maybe help find the problem? I don't know a thing about "Challenge Mode", I like to play my games where I have a bit more control over their outcomes, so I'm not pulling out my hair like you're doing. I wish you good luck, I hope you find your solution. But from the images you've posted, I think imo, your first place to look is your team & league settings.
