Like has been said, don't always trust the visuals of the game. I know what you're referring to, because I've seen it too. But to my knowledge that is just a typical sac-bunt with a little visual glitch. The fact that the runner starts running on delivery of the pitch pretty much means the hitter is going to get the bunt down (again, a visual glitch). When you see that visual, I don't believe the hitter ever misses the bunt and the catcher throws through.

Same thing on long flys. You can typically tell if its an out or a homer since runners don't advance at all on fly-outs (unless they tag after the catch of course) and run full speed on homers.
