There are custom roster sets you can use. I think they are all for real players. You can find these in the Mods section...

As far as the league set up goes, it is entirely up to you. If you want only a few teams or you want a 40 team league you can do that. I think the game is more fun when you import your own graphics. They're are a bunch of fictional player photos you can download from the mods section.

League totals won't impact how good your team is. just the league overrall.

If you play out your games then do a search for what types of situations you should use. (ie. man on 3rd less than 2 outs, you want to Pitch-->Infield In. Search for "Infield In" or something and that should help.
