There are a couple of possibilities - the easiest is probably to create the real MLB, then go to League Settings under the league menu at the top, then go to Edit League Structure and move or rename the teams to your heart's content. You can do the same with the minors, if you wish to enable them.
At that point you can release all players and hold a free agent draft, or just move the players to where you want them.
