Let me see how many of these I can address:
1) The rankings system will take into account a fighters most recent fights, the level of his opponent, and the type of win - although the latter will have a minor affect, compared to the win itself. The behind the scenes algorithm is very complete and the level of competition factor is huge, as it should be.
2) Fighters can be ranked differently by various organizations. Therefore, your #1 ranked fighter in the WBA, might be #4 in the WBC and #7 in the WBO.
3) If you choose to fight a fighter out of his weight class and want him ranked in the new weight class, you'll need to move him there - i.e. have a copy of the fighter in that weight class. If you want him to simply fight out of his weight class but retain only one ranking -- that in his primary weight class -- then there's no problem.
4) As far as your last question goes, if a fighter comes in cold from another division and beats the champion in a title fight, that fighter will become the new champion.
