I've noticed this over several versions, but it still seems to be an issue, minor as it may be. Here are the steps to recreate:

1. Create a league with one sub league and no divisions.
2. Disable the playoffs.
3. Sim to end of season. HOF voting will occur.
4. Sim additional seasons. No further HOF voting will occur.

In my testing it appears that disabling the playoffs is the trigger. If the playoffs are re-enabled HOF voting begins again.
