You can change out which team is linked by doing the following:
1. Go to the Drops page on Twitch, click on "manage your connections" in the top paragraph, then scroll down to "OOTP Perfect Team Drops" and click "disconnect".
2. Go to challenge.ootpdevelopments.com and log in with the username
of the Perfect Team account you just disconnected. Once there, click on the "Twitch" tab, and do not do anything.
3. Log out of the Challenge Mode account you're logged into on the website, and then log in to the site with the username
of the Perfect Team account you want to connect.
4. Go to the Twitch tab of the new account on challenge-dot-ootp, then click "Link with Twitch account," and it should take you to Twitch to link with your Twitch account.
The key part is step 2. Don't know why it's like that, but it doesn't work if that specific step is skipped.
