Did you alter the playoff format midseason? When I've altered my format it's always been in the offseason or preseason - that seems like the safest time to do it.
I'm sure that someone on the discussion forum can walk you through how to fix it - I've seen others run into the same kind of issues.
I think I'm going to go back to the Division Series being best-of-five... I like the sense of urgency that length of series gives without it being too short. I've experienced lots of drama when I had that format. I'll stick to the Union Championship Series being best-of-seven, and for the Pro Cup Series as well.
