Pitches in an inning as also a factor as well as overall pitch count.
Throwing 40 pitches in an inning vs 40 pitches over 4 innings (and all the mini-breaks in between) is a big difference to stamina.
Edit: And that's not counting any mental fatigue, etc.

I don't know if they model this, but I hope so. When you see anybody get to 20 pitches in real life, if there are 0-1 outs you should see somebody on the red phone. And at 30 there should probably be someone throwing unless the current guy is supposed to be taking one for the team. Or it's Bumgarner or someone like that.
