Ooh, I've got a good one for you. All of the same-state teams are in different leagues (except for California - too many teams in that state) and it somewhat makes sense geopgraphicaly and it addresses most of my little issues from my last attempt:

AL East
Boston
New York Yankees
Baltimore
Charlotte
Atlanta
Tampa Bay
(still get to keep NY-Boston and Charlotte-Atlanta as rivalries)

AL Central
Toronto
Cleveland
Pittsburgh
Detroit
Chicago White Sox
Kansas City
(Kansas City instead of Milwaukee or Minnesota so the Missouri teams can be in separate leagues)
(Cleveland-Detroit and Cleveland-Pittsburgh will be nice rivalries)

AL West
Seattle
Portland
Oakland
Anaheim
San Diego
Texas
(Seattle-Portland, definitely)

NL East
Montreal
New York Mets
Philadelphia
Washington
Miami
Havana
(Havana-Miami, for sure!)

NL Central
Chicago Cubs
Indianapolis
Cincinnati
Milwaukee
Minnesota
St. Louis
(Chicago-St. Louis, of course, but also Cincinnati-Indianapolis)

NL West
San Francisco
LA Dodgers
Arizona
Las Vegas
Colorado
Houston
(LA-SF) (LV-AZ)

Ding! Ding! Ding! We have a winner! (the only thing is you have to get used to having some long-time AL and NL teams switch leagues. But if Pittsburgh is going to be in the AL, that shouldn't be *too* hard to take.)
