We still haven't been able to find the cause. Once the OOTP team has some time (their release is coming up), we're going to get them to have a look and see if they can come up with anything. In the meantime, it's not happening consistently, so restarting the game (and deleting any already-broken portraits from \graphics\player_pics) will fix the problem.
