Historically, 'middle of the order' has meant third or fourth. Batting fifth meant you were the third best hitter, at best (perhaps behind the on-base only guys) and likely to be the target of bean balls after the third and/or fourth guys homered. As Ralph Kiner (a quintessential clean-up batter) used to say on Met broadcasts, "Poor Wally Westlake. He always batted behind me."
